Coach (NAME A) of (Company X) agrees to offer coaching services to Client (NAME B). NAME A and NAME B are required to abide by all set policies and procedures as outlined in this agreement.

DISCLAIMER

NAME B understands that NAME A is not a therapist and has not promised to act like one or provide psychological counseling.

PAYMENT OF FEES

(NAME B) shall pay ($X AMOUNT) for three months of coaching. This will include two one-on-one coaching calls monthly. The client may request a refund within the first month of payment if they wish to.

Payment has to be made on the specific date indicated on the invoice.

TERMINATION AND RENEWAL OF CONTRACT

The client understands that a relationship does not exist beyond this contract. If both parties wish to continue with the relationship, a new contract needs to be signed.
